Crossed layers, rope overlap, groove escape or crushing on the drum can shorten rope life and compromise safe operation. Damaged rope should not remain in service without competent inspection and the equipment should be placed in a safe condition.

Possible causes include a worn rope guide, incorrect winding direction, damaged drum grooves, unsuitable rope diameter or construction, insufficient installation tension, excessive fleet angle, sheave misalignment, incorrect limit settings, slack rope or load sway. Drum and sheave surfaces must also be inspected.

Rope replacement requires more than matching diameter. Construction, grade, core, end termination, minimum breaking force and manufacturer requirements must be verified. Reeving, limits and functions should be checked after installation.
